This office accepts payment in the form of cash, check, Visa, MasterCard, Discover, American Express, and CareCredit cards. Payment is expected at time of service.

VEHICULAR ACCIDENTS / JOB INJURIES

We accept and will file your Vehicular Personal Injury Protection (PIP) insurance. In addition we also accept "on-the-job" Workman Compensation injury claims. Should injury claims due to personal Injury, vehicular, or workman compensation fail to be accepted or they are denied, payment of all the medical treatment provided by this office remain the full responsibility of the patient or their guardian.

REGULAR HEALTH INSURANCE

Your visit to our office may be covered under your health insurance plan. At your request, a "superbill" of your visit will be provided for you at the time of your visit or choosing which will enable you to submit it to your insurance company. Should your policy provide coverage, you insurance will send you the reimbursement directly.

We do not accept or bill government insurance such as Medicaid, Oregon Health Plan (OHP).
